15-20 minute phone consultation---free (one time only)
This is an opportunity for me to understand your reasons for seeking therapy and it is a great time to address any initial questions or concerns you might have.

45-60 minute Intake & Assessment Appointment---$135.00 (Rate will increase to $150 beginning March 1, 2024)
This appointment is for me to understand the reasons for which you are seeking therapy, gather information needed to provide appropriate diagnosis, formulate initial treatment goals, and to begin creating a mutually agreed upon treatment plan.   

Returning Clients

45-60 minute individual therapy appointment--$125.00 (Rate will increase to $140.00 beginning March 1, 2024)

Paperwork Completion--Pro-rated based on $125.00/hour rate (Rate will increase to $140.00 beginning March 1, 2024)

Appointment Cancellations/Missed Appointments Fee--$125.00 (Rate will increase to $140.00 beginning March 1, 2024)
*To avoid a charge for appointment cancellations I ask that clients provide at least 24 hours notice of the need to change or cancel an appointment.
*Clients are permitted 1 free missed appointment/late cancellation per year. 

Care coordination Services--Pro-rated based on $125.00/hour rate (Rate will increase to $140.00 beginning March 1, 2024)

​Insurance

I currently accept Premera Blue Cross, LifeWise Health Plan of Washington, First Choice Health Network PPO and First Choice Health Network Employee Assistance Program (EAP) plans, Medicare (participating provider), Regence Blue Shield, Bridgespan, Blue Cross & Blue Shield EPO/PPO plans, & Group Health HMO/PPO plans.  

As the provider, I reserve the right to verify insurance coverage prior to your appointment.   Please bring your insurance cards with you to the first appointment and alert me if you anticipate any changes in insurance coverage or have experienced a change in your insurance coverage.

Out-of-pocket expenses for each client may vary based on the insurance plan.  Co-pays and deductibles are due to me, the provider, at time services are rendered.  Reimbursement to me, the provider, from the insurance company may vary from plan-to-plan and such fees paid to me by a client's insurer are determined by the insurer in their contract with me. Please note, that prior authorization or a referral from a physician may be required for your insurance company or your particular plan.  It is important to be aware of your benefits and how they work with respect to receiving outpatient psychotherapy services.  I encourage you to contact your insurance company directly to obtain clarification about your benefits prior to making an appointment. To do this, contact the customer services department for your insurer.  This number can generally be found on the back of your insurance card. 
 
Accepted Payment Methods

For any expenses due at time services are rendered, clients may pay with personal check made out to, "Nicole T. Shiraev, LICSW, PLLC" or clients may pay using VISA, MasterCard, American Express, Discover Card, HSA cards.
